Nawakwa Lodge 2016 Annual Pass Purchase Form

This annual pass includes payment for one year of dues as well as member registration for the three 2016 service weekends and the 2016 Fall Fellowship weekend. Separate, on-time registration is required for the four event weekends (Brotherhood program fees and Friday dinner fees are extra). If an annual pass purchaser has already paid dues for 2016 or later, an additional year of dues will be recorded at the time of purchase.

This annual pass is only valid for the single individual listed on the purchase…no transfers, no substitutions, and no refunds for unattended or partially-attended events. Registration information for lodge events will be available at the lodge website:  www.nawakwa.org
